Initial Consultation (60 minutes) | $450
Our comprehensive initial visit allows us to take a deep dive into your health goals. New patients may expect to obtain lab work following this appointment.
First Follow-Up (60 minutes) | $350
During your first visit following the initial consultation, you can expect to engage in a thorough review of your labs and other test results, along with further development of your treatment plan.
Follow-Up (30 minutes) | $175
Typical subsequent visits will last up to 30 minutes to discuss further treatment plans, new prescriptions, and/or referrals.

A: We accept cash, check, and all major credit cards. We ask that our patients pay the full balance upon service.

A: Due to the nature of extensive care in this practice, we do not accept any form of medical insurance, including Medicare and Medicaid. If you are enrolled in Medicare Part B, please complete the Medicare opt-out form on the portal. You may seek reimbursement from your insurance carrier if you have out-of-network benefits. We will provide you with an itemized bill with diagnostic ICD-10 codes and procedural CPT codes for you to file a claim with your insurance company yourself. Prior to your initial consultation, you will be required to sign this contract in agreement that you are responsible for payment of services rendered.

A: Patients with Medicare or Medicaid (even if it is secondary) cannot submit for reimbursement. Prior to your appointment, you will be asked to complete and sign the Medicare opt-out form.

A: To simplify the process, we recommend calling your insurance company prior to your visit to find out about what your specific plan will cover. We’re more than happy to provide insurance codes for registered new patients.

A: We have found that some insurance companies request additional information that is very specific and specialized to that provider. Please note that given the large number of discrepancies between what each company requires and the time needed to meet those requests, we do not assist with insurance reimbursement above and beyond the diagnosis, test codes, and receipt given at checkout.
